Recent years have seen a spurt in the use of laser in dermatology for treating skin conditions like vascular lesions, for cosmetic facial resurfacing and tattoo removal.
First developed in 1960, ‘LASER’ is an acronym for ‘light amplification by stimulated emission of radiation’. These are sources of high intensity light that have been increasingly used in the recent years in dermatology, to treat various skin disorders. It uses a high intensity beam of a particular wavelength, with negligible dispersion in a short range and focuses it on the skin. There are three components of the skin which absorb the laser light namely the melanin, hemoglobin and intracellular or extracellular water. The laser beam causes precise tissue destruction of the focused lesion without harming the surrounding skin.
Types of Lasers Used in Dermatology
There are various kinds of lasers being used, each with its own special utility. The first lasers to be used 40 years ago, used carbon dioxide and argon to treat vascular marks, such as port wine stains and haemangiomas. With introduction of newer technologies, subsequent lasers used in dermatology used the quasi-CW mode and pulsed laser light. The type of laser used for any clinical application depends on the wavelength peaks of the laser light, the pulse durations and absorption by the target skin.
Some commonly used lasers are:
- Carbon Dioxide laser
- Nd-YAG laser
- Pulsed dye laser
- Diode laser
- Argon laser
- Ruby laser
- Erbium YAG laser
Skin Conditions Treated by Laser
Vascular Lesions: The most common use of laser in dermatology is in the treatment of vascular lesions using pulsed dye laser. It was used primarily for treating port wine stains in children. However, with modifications of the initial parameters which allow longer pulses and wavelengths, it is increasingly being used in the treatment of blood vessel diseases and also for epidermal cooling. Smaller superficial vascular malformations respond better than treatment of deeper, larger blood vessels with an approximate 80% fading after 8 to 10 treatments.
Pigmented Tattoos and Lesions: An important cosmetic laser treatment is the removal of benign tattoos and pigmented lesions while minimizing the scarring and other complications. Use of Q switched lasers, ruby, Nd-YAG and alexandrite lasers causes the destruction of the melanin or the tattoo pigments for subsequent removal of the macrophages. The most common pigmented lesions that can be corrected include freckles and birthmarks as well as, blue naevi, Becker naevi. T congenital melanocytic naevi and the naevi of Ota/Ito. While Q lasers are effective in removing tattoos, it does require multiple treatments and some colors might not respond or become darker leading to complications while removing. As with other treatments using laser, it might also lead to pigmentation and scars.
Hair Removal: Unwanted hair can be effectively removed or reduced using lasers such as the long range ruby or Nd-YAG. These lasers target the melanin within the hair shaft and bulb causing the destruction of cells. This results in the slowing down the ability of hair growth.
Facial Resurfacing: Laser resurfacing allows the collagen in the skin to be rebuilt, making a patient look younger by reducing the wrinkles in the skin. Over the years erbium:YAG and carbon dioxide laser have been used, however recently the noninvasive devices have been developed. The most commonly used noninvasive device, LED or light emitting diodes have been used to stimulate the fibroblasts in the skin to produce collagen, elastin and glycosaminoglycans to rejuvenate the skin.
Lasers have also been used to remove viral warts, acne and psoriasis. Keloids and hypertrophic scars, which have been traditionally difficult to remove, can be eradicated by vaporising lasers like carbon dioxide and erbium:YAG.
It is important that the personnel using laser devices should be thoroughly trained and the patients and the clinic staff be given eye protection. Patients should keep in mind that laser treatments are basically burns, which can cause temporary pain, infection and redness. Understanding the inherent risks and complications is crucial. Comedogenic is an important word to know when dealing with acne and cosmetic products. Its definition is “tending to produce or aggravate acne”. Therefore, a comedogenic ingredient or product is one that will serve to aggravate your acne problems.
For this reason, it is a good idea to read the label of your skin care and cosmetic products before buying them. If you find any comedogenic components, stay away from that product. If you have already purchased a product containing these types of components, throw it out.
Acne Formation Our skin’s sebum is complex, made of six or seven principal components, including triglycerides and squalene, a precursor of cholesterol. The triglycerides are degraded on the surface of the skin by bacteria to free fatty acids, which cause acne episodes in genetically predisposed families. As we shall see, many cosmetics also include these acids. Unfortunately, some cosmetics include esters of fatty acids such as isopropyl mystrate or butyl stearate which are more likely to boost the production of some acne lesions. These cosmetics can interfere with traditional adult acne treatments.
Before we discuss cosmetic components, we should consider the purpose of sebum. We could assume that sebum is vital to our skin’s health. However, there is no evidence to back up that statement. Apparently, the discharge of sebum is simply a vestige whose usefulness has been lost in the path of evolution. Sebum does not avoid wrinkles either. Wrinkling, or aging of the skin, is a reflection of accumulated sun exposure, and no moisturizer on the market is capable of reversing this exhaustion of tissues.

Harmful Ingredients in Acne Treatments Three main categories of offending components
Lanolins Lanolin is, perhaps, the most common ingredient in cosmetics. Lanolin is simply sheep skin oil extracted from wool. The fatty acids in lanolin, just like fatty acids in our own oil, tend to aggravate some acne in the skin of individuals with genetic tendency towards acne. Many lanolin derivatives commonly being used in cosmetics are harmful to acne-prone individuals, i.e., etoxylated lanolins and acetylated lanolins. This particular penetrating oil is so aggressive that if left over night in a beaker, it will actually move over the top, down the borders and onto the table top. There are several chemicals analogous to isopropyl myristate in cosmetics. The most common are: decyl oleate, myristyl myristate, octyl sterate, isopropyl palmitate, putty sterate, octyl palmitate, isopropyl isothermal, isostearyl neopentonate and isocetyl stearate and PPG myristyl propionate. All must be avoided, as must other surfactants such as laureth-4.
Drugs As Acne Treatments: Maybe one the most troublesome recent findings about acne-causing potential is the red colorations used in blushes. Some of the red dyes used in drugs and cosmetics are comedogenic. This discovery is not surprising considering they are coal tar derivatives. Ever since doctors noticed that acne was an occupational illness of chimney sweeps, coal tar has been known for its acne-causing properties. An acceptable substitute for red color is carmine, a dye derived from insect wings and discovered by the Aztecs.

Antibiotics are another common treatment used in acne skin care. Like Benzoyl Peroxide, they target the P. acnes bacterium, and can be found in gels and creams, but are usually administered as an oral medication. Common antibiotics used for acne treatment are Erythromycin, Clindamycin, and Tetracycline. Another aspect of antibiotic use in acne skin care is their anti-inflammatory effect, which reduces the redness and swelling of acne eruptions that have already occurred.
Additionally, Sodium Sulfacetamide and Azelaic Acid are prescribed to fight P. acnes bacteria, although neither are antibiotics.
Azelaic Acid, Salicylic Acid, Resorcinol, sulphur, and Adapalene are products that approach acne skin care with the intent to reduce clogged pores by peeling or sloughing off dead skins cells. Salicylic Acid is found in over-the-counter acne care products, such as Stridex, as are Resorcinol and sulphur.

While most acne skin care treatments in this group target the surface of the skin, Adapalene, a prescription-only medication, reduces the build-up of dead skin cells inside the pore itself. Also a prescription-only skin care product, Azelaic Acid has the added benefit of killing P. acnes bacteria.
To reduce oil production, there are two common acne skin care treatments: oral contraceptives, and Isotretinoin (Accutane). Of the two, Accutane, a vitamin A derivative, has the most severe side-effects, which include a high risk of birth defects, and is used for severe cases when other acne skin care regimens have failed.
Oral contraceptives work as an acne treatment because they are anti-androgens—they reduce levels of male hormones, specifically testosterone. Increased oil production in women has been linked to elevated levels of testosterone. Not all birth control pills work in the same manner, and the progestins used in some actually act like androgens, so be sure to inform your doctor if you wish your oral contraceptive to serve the dual purpose of acne skin care.
Every acne skin care product available today has side effects, some of them quite severe. Always consult your doctor or dermatologist, and read carefully all labels and warnings before beginning an acne skin care regimen.
